    Evidence is insufficient. NAD+ is the central electron carrier in glycolysis, the citric acid cycle and oxidative phosphorylation, so the bioenergetic rationale is sound - ATP production genuinely depends on NAD+ cycling. But human trials tell a limited story: NMN at 250-1,000 mg/day for 6-12 weeks reliably raises blood NAD+, and one trial in amateur runners reported improved aerobic capacity thresholds at higher doses, while a trial in older adults found improved walking speed. Subjective energy, vitality and fatigue have not improved consistently, and no trial has demonstrated increased ATP production or mitochondrial output in human tissue. In people without NAD+-limited metabolism, the pathway is not rate-limited by NAD+ availability, which is the likely reason nothing is felt.

    Caveats

    Persistent fatigue deserves investigation rather than a longevity supplement: iron deficiency, B12 or folate deficiency, hypothyroidism, diabetes, sleep apnoea, depression, coeliac disease, chronic kidney disease and medication effects are common, testable and treatable. Fatigue with weight loss, night sweats, breathlessness or lymph node swelling needs prompt medical review. Sleep, exercise and reduced alcohol do more for energy than any supplement. Regulatory note: the US FDA has determined NMN is excluded from the dietary supplement definition, and availability varies by country. Safety: human trials up to 1,000 mg/day over 6-12 weeks report good tolerability, with occasional nausea, diarrhoea, headache and flushing. There is no established upper intake level and no long-term human safety data. A theoretical concern: NAD+ supports cell proliferation, and preclinical work raises the possibility that NAD+ precursors could support tumour growth - anyone with a cancer history should discuss NMN with their oncologist. Interactions are poorly characterised. Avoid in pregnancy and breastfeeding, where no safety data exist. NMN is also expensive relative to any demonstrated benefit.
