    NMN is converted to NAD+, a cofactor for sirtuins and PARPs involved in DNA repair and metabolic regulation. NAD+ declines with age, which is the basis for the ageing hypothesis.

    Evidence is insufficient. Epigenetic clocks - Horvath, GrimAge, PhenoAge, DunedinPACE - estimate biological age from DNA methylation patterns and correlate with mortality and disease risk at population level. No randomised trial of NMN has reported a reduction in epigenetic age. Human NMN trials at 250-1,000 mg/day over 6-12 weeks measured blood NAD+, insulin sensitivity, aerobic capacity and walking speed, not methylation. Two further problems apply: the clocks were built to predict age and outcomes, and it has not been established that lowering a clock reading by intervention changes the underlying biology or improves outcomes; and consumer epigenetic age tests have substantial technical variability, with repeat samples from the same person often differing by years, making individual before-and-after comparisons largely uninterpretable.

    What the studies say

    Small human trials confirm NMN raises blood NAD+ and show modest improvements in insulin sensitivity and walking performance in older adults. No randomised trial has measured methylation age as a primary endpoint, and no human longevity data exist.

    Caveats

    Regulatory status is contested in some markets. Long-term safety beyond a year is unstudied. Caution advised with active malignancy given NAD+ dependence of tumour metabolism.
