Nicotinamide mononucleotide is one step from NAD+ in the salvage pathway, converted by NMNAT enzymes inside the cell. Interest in it comes from the well-documented age-related decline in tissue NAD+ and from mouse work in which NMN improved insulin sensitivity, mitochondrial function and vascular health.
Human trials now number a couple of dozen and are consistent on one point: oral NMN raises whole-blood NAD+ in a dose-dependent way, roughly doubling it at 250-900 mg/day within a few weeks. What that buys is less clear. Trials in prediabetic postmenopausal women found improved muscle insulin sensitivity; others reported small gains in walking distance, aerobic capacity in amateur runners, and subjective drowsiness scores. Effects on body composition, blood pressure and cognition have been null or inconsistent.
Regulatory status is worth knowing: the US FDA has taken the position that NMN is excluded from the dietary supplement definition because it has been investigated as a drug, so availability varies by market. Safety in trials up to 900 mg/day for 12 weeks has been good.
