Pterostilbene is a stilbene polyphenol found mainly in blueberries and Indian kino tree heartwood. Structurally it is resveratrol with two methoxy groups in place of hydroxyls — a small change that dramatically slows liver metabolism. Where resveratrol is largely conjugated and cleared before it can act, pterostilbene reaches substantially higher and longer-lasting blood levels from the same dose, which is the main reason it has displaced resveratrol in many formulations.
The strongest human data comes from a randomised, placebo-controlled trial in adults with elevated cholesterol, where 125 mg twice daily lowered LDL cholesterol and raised blood pressure slightly in the non-statin group — a mixed result that argues for monitoring rather than enthusiasm. Other small trials have reported reductions in oxidative stress markers. Cognitive and longevity claims rest on rodent and cell work involving SIRT1 activation and Nrf2-mediated antioxidant signalling; they have not been demonstrated in people.
Pterostilbene is best understood as a promising metabolic and antioxidant compound with a thin but real human evidence base. It is a reasonable adjunct for someone already managing lipids and blood sugar through diet and exercise, and a poor substitute for either.
