The cognitive rationale is entirely preclinical: pterostilbene crosses the blood-brain barrier more readily than resveratrol, and rodent studies show improved working memory in aged animals, reduced hippocampal oxidative stress, modulation of dopamine signalling, and reduced amyloid pathology in Alzheimer models. Joseph and colleagues found pterostilbene improved cognitive performance in aged rats and correlated it with hippocampal accumulation. No human trial has measured memory, attention, processing speed or any neuropsychological outcome after pterostilbene, at any dose or duration.

    Rodent cognition studies are a weak basis for human claims - the translation rate from aged-rat memory findings to human cognitive benefit is very poor. For cognitive health the evidence sits with physical exercise, sleep, treating hearing loss, controlling blood pressure and diabetes in midlife, not smoking, limiting alcohol and social engagement. Progressive memory problems or cognitive change affecting function need medical assessment, since B12 deficiency, hypothyroidism, depression, sleep apnoea and anticholinergic medication burden are common reversible contributors. Safety of pterostilbene: the key flag is that 250 mg/day increased LDL cholesterol in the Riche randomised trial - relevant because vascular risk factors drive cognitive decline, so raising LDL works against the goal. Human safety data extend to roughly 8 weeks only, so long-term safety is unknown, and no upper intake level has been established; do not exceed the studied range. Pterostilbene inhibits cytochrome P450 enzymes in vitro and may have antiplatelet activity, so use caution with anticoagulants, antiplatelet drugs and narrow-therapeutic-index medication, and stop before surgery. Avoid in pregnancy and breastfeeding, since no safety data exist.
