NAD+ is genuinely central to cellular function - it is the electron carrier in glycolysis and oxidative phosphorylation, the substrate for sirtuins and PARP enzymes involved in DNA repair, and its levels decline with age in multiple tissues. NMN supplementation at 250-1,000 mg/day reliably raises blood NAD+ in human trials lasting 6-12 weeks. The gap is between that biochemistry and any demonstrated cellular or clinical benefit: no human trial has shown improved mitochondrial function, reduced senescent cell burden, improved DNA repair capacity or any downstream cellular outcome. Raising a metabolite is not the same as improving cell health, and the assumption that more NAD+ is uniformly better has not been tested.

Caveats
Cellular health is a marketing frame with no corresponding test, and should not be the basis for buying a supplement. NMN raises NAD+; everything beyond that is extrapolation from mouse and cell culture work. Regulatory note: the US FDA has determined NMN is excluded from the dietary supplement definition, and availability differs by country. Safety: trials up to 1,000 mg/day over 6-12 weeks report good tolerability, with occasional nausea, diarrhoea, headache and flushing. There is no established upper intake level and no long-term human safety data. A theoretical concern worth stating plainly: NAD+ supports cell proliferation and DNA repair, processes that cancer cells also use, and preclinical work raises the possibility that NAD+ precursors could support tumour growth - anyone with a current or previous cancer should discuss NMN with their oncologist before use. Interactions are poorly characterised, and NMN is often sold alongside resveratrol and other compounds with their own interaction profiles. Avoid in pregnancy and breastfeeding, where no safety data exist. Exercise remains the intervention with the best evidence for mitochondrial function.
