    Raising NAD is the one thing NMN has clearly been shown to do in humans.

    The question of whether NMN actually raises NAD+ in humans has a clearer answer than most of the longevity field. In 42 healthy older Japanese men, 250 mg daily for 12 weeks raised whole-blood NAD+ metabolites versus placebo.
    The same trial also found improved gait speed and right-hand grip strength, while muscle insulin sensitivity and most other metabolic measures did not differ significantly. That split is the honest summary: the biochemical endpoint moved reliably, the functional endpoints moved partially, and the metabolic endpoints mostly did not. So NMN does what it claims at the level of NAD+ chemistry. Whether raising blood NAD+ metabolites translates into the health outcomes the category is marketed on remains unproven, and one small trial in one demographic cannot settle it.

    How It Works

    NAD+ is the electron carrier at the centre of energy metabolism and the required cofactor for sirtuins and PARP enzymes. Tissue NAD+ declines with age, which is the premise the entire category rests on.
    NMN sits one enzymatic step from NAD+, converted by NMN adenylyltransferase. There has been genuine scientific argument about whether oral NMN is absorbed intact or dephosphorylated to nicotinamide riboside first; the trial evidence shows blood NAD+ metabolites rise either way, so the route matters less than the outcome. The unresolved question is compartments. Whole-blood NAD+ metabolites are what was measured, and they are not the same as NAD+ inside muscle, brain or liver mitochondria, which is where the proposed benefits would have to originate.
