- Evidence is insufficient. Human trials of NMN at 250-1,000 mg/day, lasting 6-12 weeks, consistently show it raises blood NAD+ levels and is well tolerated - that much is established. What has not been shown is any effect on ageing itself. The trials measured surrogate outcomes: NAD+ concentration, insulin sensitivity in a small subgroup of prediabetic women, aerobic capacity in amateur runners, and walking speed. None measured mortality, disease incidence, frailty progression or any validated ageing endpoint, and none ran longer than a few months. Mouse studies showing NMN improves metabolic and physical parameters in aged animals are the source of the enthusiasm, and mouse ageing results have a poor record of translating to humans.

Caveats
Anti-ageing is not a measurable clinical outcome, and no supplement has been shown to slow human ageing. NMN raising NAD+ is real; that this translates to slower ageing is an untested assumption. Note the regulatory position: the US FDA has determined NMN is excluded from the dietary supplement definition because it was investigated as a drug, and availability varies by country. Safety: human trials up to 1,000 mg/day for 6-12 weeks report good tolerability, with mild nausea, diarrhoea, headache and flushing occasionally. There is no established upper intake level and no long-term human safety data - trials of this length cannot exclude effects that matter over years. A theoretical concern deserves stating: NAD+ availability supports cell proliferation, and some preclinical work suggests NAD+ precursors could support tumour growth or, through NAD+-dependent enzymes, affect cancer biology - people with a cancer history or active cancer should discuss NMN with their oncologist. Interactions are poorly characterised. Avoid in pregnancy and breastfeeding, where no safety data exist. The interventions with actual evidence for healthy ageing remain exercise, not smoking, blood pressure control, sleep and diet.
