    Evidence is insufficient, and structurally so. Human NMN trials have lasted 6-12 weeks and measured blood NAD+, insulin sensitivity, aerobic capacity and walking speed - trials of that length cannot address lifespan under any circumstances. The enthusiasm derives from mouse work, where NMN improved metabolic parameters, physical activity and some age-related measures in aged animals; notably, even in mice the lifespan evidence is inconsistent, and the related precursor nicotinamide riboside did not extend lifespan in the NIA Interventions Testing Program, which is the most rigorous mouse longevity testing programme. Mouse longevity results translate poorly to humans generally, and no intervention has ever been shown to extend human lifespan in a randomised trial.

    Human RCTs confirm NMN raises blood NAD+ and shows signals (insulin sensitivity, muscle performance, arterial stiffness in some trials), but results are inconsistent and no clinical endpoint data exist. Regulatory status in the US shifted (FDA excluded it from supplements due to drug investigation) — availability varies.
