- Biotin is a cofactor for carboxylase enzymes in energy metabolism, so deficiency impairs it. But overt deficiency is rare, and no controlled trial demonstrates energy or fatigue benefit from supplementation in healthy adults

- Persistent fatigue warrants testing for iron deficiency, thyroid disease, sleep apnoea, depression and B12 deficiency - all common and all treatable

- Stop biotin 3 days before those tests, since it distorts thyroid results

Safety
Caveats
Biotin interferes with immunoassays: falsely low TSH, falsely high free T4 and falsely low troponin. It can make you look hyperthyroid on paper and can mask a heart attack. Stop at least 72 hours before blood tests. Deficiency risk is confined to long-term anticonvulsant use, chronic alcohol use, raw egg white consumption and biotinidase deficiency. Standard doses are safe in pregnancy; megadoses are not established.
