    Evidence is insufficient. The energy claims for PQQ derive from preclinical work showing it upregulates PGC-1alpha and mitochondrial biogenesis in cells and rodents, plus a small Japanese study reporting improvements in self-rated fatigue, sleep quality and vigour at 20 mg/day. That study was small, poorly described methodologically and has not been independently replicated in an adequately powered trial. No human study has measured ATP production, resting metabolic rate, exercise capacity or objective fatigue endpoints after PQQ supplementation. Subjective energy is also among the most placebo-responsive outcomes in supplement research, which makes small unblinded findings particularly unreliable.

    Caveats

    Persistent fatigue deserves a diagnosis rather than a supplement. Common and treatable causes include iron deficiency with or without anaemia, hypothyroidism, sleep apnoea, depression, diabetes, coeliac disease, vitamin B12 or D deficiency, chronic kidney disease, and medication effects from beta-blockers, antihistamines, opioids and antidepressants. Fatigue with weight loss, night sweats, breathlessness, lymph node swelling or bleeding needs prompt medical assessment. Sleep quantity and regularity, physical activity and alcohol reduction produce far more change than any energy supplement. Safety of PQQ: the human evidence base is small and short, so long-term safety is unknown - reported side effects at 20-40 mg/day include headache, fatigue and sleep disturbance, which is a notable irony for an energy product. No upper intake level has been established for humans, and a rodent study found kidney effects at very high doses, so do not exceed the 20 mg/day studied. Avoid in pregnancy and breastfeeding, as no safety data exist. Drug interactions are undocumented rather than excluded.
