    Evidence is insufficient. Spermidine influences protein handling through two mechanisms with real cell-biological support: autophagy induction clears misfolded and aggregated proteins, and spermidine is the obligatory substrate for hypusination of eIF5A, a translation factor required for efficient synthesis of certain protein motifs. Both are well characterised in cells and model organisms, and spermidine reduces protein aggregation in animal models of neurodegeneration. No human study has measured proteostasis, chaperone activity, aggregate burden or eIF5A hypusination after supplementation - these require tissue sampling and specialised assays.

    Animal studies show lifespan extension and improved proteostasis. Human trials are small pilots with modest or null cognitive outcomes and no hard endpoints.
