Spermidine protects neurons in animal models through autophagy induction, clearing protein aggregates and improving mitochondrial quality control, and a small pilot in older adults with subjective cognitive decline suggested memory improvement. The definitive test was SmartAge, a 12-month randomised placebo-controlled trial of 1.2 mg/day in 100 older adults with subjective cognitive decline: it found no significant improvement in memory performance or other cognitive outcomes versus placebo. The dose may have been too low, but as it stands the best human trial is negative.

    Caveats

    The pattern here is common in the longevity field: promising animal neuroprotection, an encouraging small pilot, then a properly powered trial that finds nothing. That does not disprove an effect at higher doses, but it means no one should expect cognitive benefit from spermidine at studied doses. What does have evidence for cognitive ageing is physical activity, blood pressure control in midlife, treating hearing loss, not smoking, limiting alcohol, sleep, managing diabetes and staying socially and mentally engaged - the Lancet Commission modifiable risk factors. New or progressive memory problems, particularly with personality change, disorientation or difficulty with familiar tasks, need medical assessment: some causes including B12 deficiency, thyroid disease, depression and medication effects are reversible. Safety of spermidine: up to 6 mg/day appears well tolerated short term, with no established upper limit and no long-term data. Wheat germ extract contains gluten. Polyamines drive cell proliferation, so there is theoretical concern in active cancer - discuss with an oncology team. Safety in pregnancy and breastfeeding is not established.
