- Evidence is insufficient for clinically meaningful weight loss. A meta-analysis of randomised trials found carnitine supplementation at around 2 g/day produced modest weight reduction versus placebo - roughly 1-1.3 kg on average - with the effect diminishing over time and largely disappearing in longer trials, and several individual studies found no difference at all. Most trials were short, many in overweight or obese participants alongside dietary advice, and heterogeneity was high. Mechanistically the ceiling is clear: muscle carnitine is already near saturation in healthy people, oral carnitine does not raise it without a large insulin stimulus, and increasing fatty acid transport does not create an energy deficit. Weight loss requires that deficit.
