- Evidence is insufficient. Observational work links microbiome composition, particularly Firmicutes to Bacteroidetes ratio, with obesity, and rodent transfer experiments show microbiota can transmit metabolic phenotypes - a striking finding that has driven considerable enthusiasm. Human trials are underwhelming: meta-analyses show mean weight reductions of roughly 0.5-1 kg over 8-24 weeks, with high heterogeneity, small samples, frequent industry funding and inconsistent findings between strains. A Lactobacillus gasseri trial showed abdominal fat reduction that regressed after stopping. Some strains, notably certain Lactobacillus species used in agriculture, are associated with weight gain.
