    Evidence is insufficient. Some HMB trials report reductions in fat mass alongside increases in lean mass, which generated the fat-loss claim, but the finding is inconsistent: meta-analyses find no reliable effect of HMB on fat mass, the positive results cluster in trials from a single research group whose effect sizes drew formal expert criticism, and where body composition improved it was usually driven by the training programme. HMB is anti-catabolic - it reduces muscle protein breakdown - and there is no mechanism by which that would increase fat oxidation. Its plausible role is preserving lean mass during an energy deficit, which is a different claim, and even that has limited support beyond adequate protein intake.

    Meta-analyses report negligible average fat mass changes with HMB across resistance-training trials, with positive findings concentrated in a small number of outlier studies.

    Caveats

    Fat loss requires an energy deficit; the supplement question is only whether lean mass can be preserved within it, and adequate protein at 1.6-2.2 g/kg/day plus resistance training does that job well. Safety: HMB has a good safety record at 3 g/day over months in trials, with no consistent adverse effects on liver, kidney or lipid markers, but there is no established upper intake level and no long-term safety data beyond a year. Avoid in pregnancy and breastfeeding, where safety is not established, and in children and adolescents. Use caution in renal or hepatic impairment given limited data - relevant since fat-loss dieting is often combined with high protein intake. No well-characterised drug interactions, but disclose it to your clinician. Fat-loss products combining HMB with stimulants carry the risks of those stimulants, not of HMB. Athletes should use batch-tested products.
