- Take with a large carbohydrate load. Muscle carnitine uptake is insulin-dependent, and without a strong insulin stimulus oral carnitine raises plasma levels without changing muscle content at all. Evidence is mixed. The Stephens and Wall studies showed that 2 g twice daily with around 80 g carbohydrate for 12-24 weeks raises muscle carnitine roughly 20% and shifts substrate use - sparing glycogen at low intensity and increasing its use at high intensity, which is the textbook definition of improved metabolic flexibility. That is a real finding, but it comes from a small number of studies by a single group, requires an impractical daily carbohydrate load, and has not translated into consistent body composition or performance outcomes elsewhere.

Safety
Caveats
Metabolic flexibility responds far more to training - particularly zone 2 endurance work and resistance training - and to weight loss and sleep than to any supplement. Safety ceiling: doses above 3 g/day commonly cause nausea, vomiting, abdominal cramps, diarrhoea and a fishy body odour; keep below 4 g/day. Note the loading protocol itself adds several hundred carbohydrate calories daily. The TMAO concern applies to long-term use: gut bacteria convert carnitine to trimethylamine, which the liver oxidises to TMAO, a metabolite associated in prospective cohorts with cardiovascular events; significance unresolved but a reason for caution with prolonged high doses, particularly with existing cardiovascular disease. Carnitine may potentiate warfarin - monitor INR. Avoid in seizure disorders, where increased seizure frequency has been reported, and in hypothyroidism, where it may antagonise thyroid hormone action. Use cautiously in kidney disease, where carnitine accumulates. Insufficient safety data in pregnancy and breastfeeding - avoid in both.
