    L-Carnitine for Sperm Quality

    L-carnitine has consistent trial evidence for motility improvement.

    Overview

    L-carnitine has more supporting data for male fertility than most supplements sold for it. A systematic review of carnitine in male infertility found improvements in total and forward sperm motility, with some studies also reporting higher pregnancy rates.
    Motility is where the effect sits. Changes in sperm concentration were inconsistent across the same trials, so the accurate claim is that carnitine helps sperm move better rather than that it makes more of them. That distinction is useful clinically, because asthenozoospermia, poor motility with adequate counts, is the pattern most likely to respond. It also means carnitine is not a treatment for azoospermia or for a structural or hormonal cause of infertility.

    How It Works

    Sperm swim on fatty acid oxidation, and carnitine is the shuttle that carries long-chain fatty acids into mitochondria to be burned. Epididymal fluid concentrates carnitine to levels many times higher than blood, which tells you how central it is to sperm energetics.
    More available carnitine means more ATP for the flagellum, which is the direct explanation for improved forward motility. Carnitine also has antioxidant activity, protecting the polyunsaturated-rich sperm membrane from reactive oxygen species, a major cause of sperm dysfunction. Spermatogenesis takes roughly 74 days plus epididymal transit, so any intervention affecting sperm development needs three months before the semen analysis reflects it.

    Safety

    L-carnitine is well tolerated at fertility doses. The most noticeable effect is a fishy body odour in a minority of users, caused by gut bacterial conversion of carnitine to trimethylamine.
