    Carnitine shuttles long-chain fatty acids across the inner mitochondrial membrane via the carnitine palmitoyltransferase system, a rate-limiting step in beta-oxidation.

    Take with a substantial carbohydrate load. Evidence is mixed and hinges on whether muscle carnitine actually increases. Carnitine is required to shuttle long-chain fatty acids into the mitochondrion for beta-oxidation, so the rationale is textbook biochemistry. The problem is that muscle carnitine is already near saturation in healthy people, and raising it requires insulin-stimulated uptake - the well-known Stephens and Wall studies showed that 2 g twice daily with substantial carbohydrate over 12-24 weeks does raise muscle carnitine by around 20% and shifts fuel use, sparing glycogen at low intensity, whereas carnitine alone does not change muscle content at all. Most commercial use ignores this, which explains the largely negative literature and the absence of reliable fat loss.

    What the studies say

    Muscle carnitine is difficult to raise; studies needed 80 g of carbohydrate alongside 2 g carnitine over 12-24 weeks to increase muscle content. Meta-analyses show small weight loss effects, mostly in obese or older populations. Healthy trained people show no change in fat oxidation.

    Safety

    Caveats

    Gut bacteria convert carnitine to TMAO, which has been associated with cardiovascular risk. GI upset and fishy body odour at higher doses.
