    Evidence is insufficient. The rationale rests on tongkat ali modest effects on testosterone in hypogonadal or stressed men, plus a small number of exercise studies. Results are weak and inconsistent: an early study by Hamzah and Yusof reported increased lean mass and arm circumference in men doing resistance training, but it was small and poorly controlled, and subsequent trials in trained and older participants have generally found no meaningful improvement in strength, power, endurance or body composition versus placebo. Sample sizes are small, protocols vary, and several studies are industry-funded.

    Small studies report improved muscular strength or endurance in older or physically active adults, but sample sizes are under 30 and independent replication is lacking.

    Performance supplements with real evidence are creatine monohydrate at 3-5 g/day, caffeine at 3-6 mg/kg, beta-alanine at 3-6 g/day for efforts of 1-4 minutes, and nitrate from beetroot juice for endurance - tongkat ali is not among them, and modest hormonal changes within the normal range do not produce measurable performance effects. Safety of tongkat ali: generally well tolerated in trials at 200-400 mg/day, with insomnia, restlessness and irritability reported. Contamination is a real concern - Malaysian surveys have found high lead levels in a substantial proportion of commercial tongkat ali products, so use only third-party tested standardised extracts. It may lower blood glucose, relevant in diabetes, and may affect blood pressure medication. Avoid in hormone-sensitive cancers including prostate and breast cancer, and in men with prostate disease. Avoid in pregnancy and breastfeeding. Long-term safety beyond a few months is not established. Competitive athletes should use third-party certified products, since testosterone-marketed supplements are a common source of contamination.
