Tongkat ali is proposed to raise free testosterone by displacing it from sex hormone binding globulin rather than by increasing production, and Talbott and colleagues reported that 200 mg/day for 4 weeks in moderately stressed adults reduced cortisol by 16%, increased testosterone by 37% and improved the testosterone-to-cortisol ratio. Other trials in hypogonadal men reported increases in total testosterone. Effects are largest in men with low baseline testosterone or high stress, small or absent in eugonadal men, and several trials are industry-funded with small samples.

    What the studies say

    Trials in stressed and hypogonadal adults report significant increases in free testosterone and improved cortisol-to-testosterone ratio. Studies in healthy young athletes generally show no meaningful change.

    Pooled analysis of randomised trials of Eurycoma longifolia (Tongkat Ali) found small improvements in erectile function scores and some testosterone-related parameters versus placebo, but effects were inconsistent across trials and largely limited to men with baseline hypogonadism or stress-related low testosterone.

    Caveats

    Chasing a testosterone number is rarely the right goal - what matters is symptoms, and testosterone within the normal range does not warrant treatment. Genuine hypogonadism requires two morning testosterone measurements, LH and FSH, and prolactin, and has treatable causes including obesity, sleep apnoea, opioid use, excess alcohol and pituitary disease; losing weight and treating sleep apnoea often raise testosterone more than any supplement. Symptoms of low testosterone - low libido, fatigue, loss of morning erections, reduced muscle mass - warrant proper endocrine assessment rather than self-treatment. Safety of tongkat ali: generally well tolerated at 200-400 mg/day, with insomnia, restlessness and irritability reported, which is why morning dosing is advised. Lead contamination has been documented in a substantial proportion of Malaysian market products - use third-party tested standardised extracts only. It may lower blood glucose and affect antihypertensives. Avoid in hormone-sensitive cancers including prostate and breast, and in prostate disease, since raising androgens is undesirable there. Avoid in pregnancy and breastfeeding. Long-term safety beyond a few months is unknown.
