- Take in the morning with food, since evening doses may disturb sleep. Evidence is mixed. The most cited trial, by Tambi and colleagues, gave 200 mg/day of standardised extract to 350 men with idiopathic infertility for up to 9 months and reported improvements in semen volume, sperm concentration and motility, with 11 spontaneous pregnancies. Animal work supports effects on spermatogenesis and testosterone. The problem is that the positive trials are largely open-label, single-centre, Malaysian and industry-linked, with few independent replications and limited placebo-controlled data.

Caveats
The evidence base is dominated by open-label industry-linked studies from a single research group, which is a substantial limitation for a fertility claim. Proper assessment matters more: a couple not conceiving after 12 months, or 6 months if the female partner is over 35, should be investigated, since varicocele, obstruction, hormonal and genetic causes need identifying and treatment success declines with delay. Stopping smoking, limiting alcohol, avoiding testicular heat, treating obesity and stopping anabolic steroids - which suppress spermatogenesis profoundly and sometimes permanently - all matter more. Safety of tongkat ali: generally well tolerated at 200-400 mg/day, with insomnia, restlessness and irritability reported. Lead contamination has been found in a substantial proportion of commercial products in Malaysian surveys, which is a serious concern when trying to conceive - use third-party tested standardised extracts only. It may lower blood glucose and affect blood pressure medication. Avoid in hormone-sensitive cancers and prostate disease. Avoid in pregnancy and breastfeeding. Long-term safety is not established.
