Keep supplemental folic acid at or below 1 mg daily unless a clinician has prescribed the higher-risk dose.
Simple protocol

Start before conception
At least one month ahead, and earlier if pregnancy could happen unplanned. The neural tube closes before most people know they are pregnant.

Take 400 micrograms folic acid daily· Preconception to 12 weeks
Continue through the first 12 weeks of pregnancy at minimum.

Ask whether you need 5 mg
A previous affected pregnancy, diabetes, high BMI, epilepsy medication or malabsorption all change the recommended dose.

Check B12 status
Folate can mask B12 deficiency, and B12 is needed for the remethylation step.

Do not use it as a fertility treatment
The large randomised trial found no improvement in semen quality or live births. Fertility problems need proper assessment.

Continue a pregnancy multivitamin as advised
Folate is one component of antenatal nutrition, alongside iodine, vitamin D and iron where indicated.
