    Evidence is insufficient in humans despite strong animal data. The mouse work by Eisenberg and colleagues is among the more credible findings in the field: dietary spermidine extended median lifespan, reduced cardiac hypertrophy, improved diastolic function and lowered blood pressure in rodents, with autophagy induction as the mechanism. Human data are observational - higher dietary spermidine intake associates with lower cardiovascular and all-cause mortality in cohort studies, which is confounded by the foods involved. No randomised trial has measured blood pressure, cardiac function or cardiovascular events after spermidine supplementation.

    What the studies say

    Mouse studies show delayed cardiac aging and reduced hypertension-induced heart failure. Human evidence is limited to observational associations between dietary spermidine and lower cardiovascular mortality.

    Safety

    Caveats

    Cardiovascular risk has interventions with outcome evidence that no supplement approaches: controlling blood pressure, statins where indicated, not smoking, physical activity, and managing diabetes and weight. Anyone with established heart disease, heart failure or hypertension should not substitute spermidine for treatment, and should not stop prescribed medication. Chest pain, breathlessness on exertion, palpitations or ankle swelling need medical assessment rather than a longevity supplement. Safety of spermidine: up to 6 mg/day appears well tolerated in short trials, but there is no established upper limit and no long-term human safety data. Wheat germ extract contains gluten and is unsuitable in coeliac disease or wheat allergy unless certified gluten-free. Polyamines drive cell proliferation and are elevated in many tumours, so there is theoretical concern about supplementation during active cancer - discuss with an oncology team first. Safety in pregnancy and breastfeeding is not established. Dietary spermidine from wheat germ, aged cheese, mushrooms, soy and legumes is the route the observational data actually describe.
