    Better absorbed, worse value. Sound for general omega-3 status if fish oil causes reflux; the wrong tool if you need therapeutic doses for high triglycerides.

    How It Works

    Phospholipid-bound EPA and DHA are incorporated directly into membrane phospholipid pools, altering the substrate available to phospholipase A2 and shifting eicosanoid production toward less inflammatory series and toward pro-resolving mediators such as resolvins. Higher membrane omega-3 content also reduces hepatic VLDL triglyceride secretion and stabilises cardiac myocyte membranes.

    Evidence

    What the studies say

    Head-to-head absorption studies consistently show krill oil raising the omega-3 index more per gram of EPA and DHA delivered than triglyceride or ethyl-ester fish oil, attributed to phospholipid emulsification that bypasses the need for bile-mediated lipase digestion. On lipids, randomised trials of krill oil at 1-4 g/day report triglyceride reductions in the range of 10-20% in people with elevated baseline values, with little effect in those already normal, plus small and inconsistent changes in HDL and LDL. The critical gap is outcomes: no cardiovascular event trial has been run on krill oil. What we have for fish oil at high dose — the REDUCE-IT and STRENGTH results, which themselves conflict — cannot simply be transferred to krill, and the doses used in those trials, 3-4 g/day of EPA and DHA, would require an impractical and expensive number of krill capsules. On current evidence krill is a good omega-3 delivery vehicle with a small absorption advantage and no demonstrated clinical superiority.

    Safety

    Caveats

    Krill are crustaceans, so it is unsafe in shellfish allergy. Per gram of EPA and DHA it costs three to five times more than fish oil. No cardiovascular outcome trial exists. Bleeding risk rises at high doses, and Antarctic krill harvesting carries genuine ecological considerations.

    Less likely to help if

    People with severe hypertriglyceridaemia needing 3-4 g/day of EPA and DHA, for whom capsule count and cost make krill impractical, and anyone whose lipid problem is driven by alcohol or uncontrolled diabetes.
