    Take with food. Evidence is mixed and heavily weighted toward preclinical work. In cell and animal studies, hericenones and erinacines stimulate nerve growth factor synthesis, and some rodent work shows increased BDNF expression and hippocampal neurogenesis - this is the basis for essentially all lion's mane marketing. The human evidence for BDNF specifically is very thin: few trials have measured circulating BDNF, results are inconsistent, and serum BDNF is in any case a poor proxy for brain BDNF. Note also that hericenones and erinacines have limited blood-brain barrier penetration data in humans. The mechanism is plausible and interesting; it is not demonstrated in people.

    Evidence

    What the studies say

    A small Japanese randomised trial in older adults with mild cognitive impairment found improved cognitive scores over 16 weeks that regressed after discontinuation. Subsequent small trials report improvements in subjective wellbeing, anxiety and processing speed. Sample sizes are typically under 60 and extract standardisation varies widely between products. The NGF mechanism is well demonstrated in vitro but human neurotrophic measurements are largely absent.

    No studies are yet linked to both Lion's Mane and BDNF Enhancement.

    Safety

    Caveats

    BDNF is a mechanism, not a health outcome, and the interventions with the strongest human evidence for raising it are aerobic exercise, sleep and calorie restriction - not a mushroom extract. Safety: lion's mane is generally well tolerated in trials at 1-3 g/day over months, with mild gastrointestinal upset the most common effect. Skin rashes and, rarely, respiratory allergic reactions have been reported, and anyone with mushroom allergy should avoid it. It may have mild antiplatelet and glucose-lowering effects - use caution with anticoagulants, antiplatelets and diabetes medication, and stop two weeks before surgery. There is no established upper intake level and no long-term safety data beyond about a year. No safety data in pregnancy or breastfeeding - avoid in both, and in children. Product quality varies enormously; look for stated fruiting body content and beta-glucan quantification rather than vague polysaccharide figures.
