- Take with food. Evidence is mixed. The preclinical case is the strongest part of the lion's mane story: in rodent models of peripheral nerve crush injury, lion's mane extract accelerates axonal regeneration and functional recovery, and hericenones and erinacines stimulate NGF synthesis in cell culture - NGF being genuinely important for sensory and sympathetic neuron maintenance. What is missing is human data. There are no adequate randomised trials of lion's mane for peripheral neuropathy, nerve injury recovery or any nerve-specific human endpoint. Extrapolating rat sciatic nerve recovery to human neuropathy is a substantial leap, and NGF itself has failed in human neuropathy trials partly because of pain side effects.

Peripheral neuropathy needs a diagnosis, not a supplement: diabetes, B12 deficiency, alcohol, hypothyroidism, chemotherapy, HIV and autoimmune causes are all identifiable and several are reversible if caught early - B12 deficiency in particular causes irreversible damage if left untreated. Numbness with weakness, bladder or bowel change, or rapid progression needs urgent assessment. Blood glucose control is the single most important intervention in diabetic neuropathy. Safety: lion's mane is generally well tolerated at 1-3 g/day over months, with mild gastrointestinal upset most common; skin rash and rare respiratory allergic reactions have been reported, and it should be avoided with mushroom allergy. Possible mild antiplatelet and glucose-lowering activity means caution with anticoagulants, antiplatelets and diabetes medication - the latter matters here, since many people with neuropathy are diabetic - and stopping two weeks before surgery. No established upper intake level, and no long-term safety data beyond about a year. No safety data in pregnancy or breastfeeding - avoid in both, and in children.
