Overview
Hyaluronic acid is a glycosaminoglycan found throughout connective tissue, most concentrated in skin, synovial fluid and the vitreous humour. It binds up to a thousand times its weight in water, which is what gives skin its turgor and synovial fluid its viscoelasticity. Body content declines with age, and by the sixties skin hyaluronic acid is a fraction of youthful levels.
Oral supplementation raises an obvious question: a molecule this large should not survive digestion intact. The answer appears to be that gut bacteria degrade it into oligosaccharides that are absorbed, distribute to skin and joints, and stimulate endogenous hyaluronic acid synthesis by fibroblasts rather than being incorporated directly. Radiolabelled tracer studies support this distribution, and low-molecular-weight preparations of around 50-300 kDa appear better absorbed than high-molecular-weight forms.
The clinical evidence is real but modest. Several randomised trials, mostly Japanese and several industry-funded, show improved skin hydration and reduced wrinkle depth at 120-240 mg/day over 8-12 weeks. For knee osteoarthritis, oral trials show small improvements in pain and function, well short of what intra-articular injection achieves. Topical hyaluronic acid works by an entirely different route — surface humectancy — and is not comparable.
