Intra-articular HA injection for knee osteoarthritis has been extensively studied, and the conclusion has moved against it - major guidelines including NICE and the American Academy of Orthopaedic Surgeons now recommend against routine use, since the benefit over placebo injection is small and within the range of bias. Oral HA has fewer and smaller trials, mostly from Japan, reporting modest improvements in knee pain and stiffness scores at 80-200 mg/day over 8-12 weeks, plausibly via effects on synovial HA production or systemic anti-inflammatory action rather than direct incorporation into cartilage. Effect sizes are small and independent replication is limited.

    Safety

    Caveats

    Weight loss and strengthening exercise outperform every supplement for osteoarthritis and should come first - each kilogram lost meaningfully reduces knee load. A hot, red, swollen joint with fever needs urgent assessment for septic arthritis, and prolonged morning stiffness over an hour affecting multiple small joints suggests inflammatory arthritis needing rheumatology input. Safety: oral HA is well tolerated at 80-240 mg/day in trials lasting months, with no established upper intake level and no long-term safety data beyond a year. Some products are derived from rooster comb - avoid with egg or poultry allergy; fermentation-derived products avoid this. Avoid in active cancer without oncology advice, given HA involvement in angiogenesis and CD44 signalling. No safety data in pregnancy or breastfeeding - avoid in both. Intra-articular injections carry infection and post-injection flare risks and must be given by a clinician.
