    Why It Matters

    Most persistent joint discomfort in adults is osteoarthritis, a whole-joint condition involving cartilage loss, subchondral bone change and low-grade synovial inflammation — not simple mechanical wear. Radiographic severity correlates poorly with symptoms, which is clinically important: many people with visible joint changes have no pain, and many with pain have modest imaging findings. That decoupling is why strength, load tolerance and body weight influence symptoms more reliably than structural measures do. Inflammatory arthritis such as rheumatoid disease is a different problem requiring different treatment, and morning stiffness lasting over an hour, joint swelling or symmetrical small-joint involvement should prompt medical assessment rather than supplementation.

    How to Measure

    Assessment is largely clinical. WOMAC and KOOS questionnaires quantify pain and function and track change over time. A 30-second sit-to-stand and timed up-and-go measure practical capacity. Blood tests including CRP, ESR, rheumatoid factor and anti-CCP distinguish inflammatory arthritis. Imaging with weight-bearing X-ray is useful for staging but should not drive treatment on its own given the weak link with symptoms.

    Optimization Protocol

    Strengthen the muscles around the joint. Quadriceps and hip strengthening for knee osteoarthritis has effect sizes comparable to NSAIDs in trials, without the gastrointestinal risk, and the benefit is durable while training continues. Lose weight if overweight: knee load falls by roughly four kilograms per kilogram lost, and combined diet plus exercise beats either alone. Keep moving — rest worsens stiffness and deconditions the joint. Among supplements, the picture is mixed but not empty: curcumin at 500-1000 mg/day with a bioavailability enhancer has repeatedly matched NSAIDs for knee pain in small trials, collagen peptides at 10 g/day have modest positive data, glucosamine and chondroitin show benefit in industry trials and little in independent ones, boswellia has small favourable trials, and omega-3s help more in inflammatory arthritis than in osteoarthritis. Heat, topical NSAIDs and graded loading are practical adjuncts.
