Rare but well-documented cases of drug-induced liver injury have been reported, predominantly with high-bioavailability formulations and often in people carrying a particular HLA genotype — stop and seek review if you develop jaundice, dark urine or unexplained fatigue. Curcumin stimulates gallbladder contraction, so avoid it with gallstones or biliary obstruction. It has mild antiplatelet activity: stop two weeks before surgery and be cautious on anticoagulants. Avoid therapeutic doses in pregnancy, and note that piperine raises absorption of many medications, not just curcumin.
