    Omega-3 Fatty Acids for Joint Comfort

    Omega-3 at 2-3 g/day reduces joint tenderness, morning stiffness and NSAID requirement in inflammatory arthritis.

    Overview

    Omega-3 fatty acids have the best-characterised joint evidence of any supplement, and the pattern is worth stating precisely: the effect is clear in inflammatory arthritis and much weaker in osteoarthritis. Rheumatoid arthritis trials consistently report reduced morning stiffness, fewer tender joints and — the most clinically meaningful finding — reduced NSAID requirement. In osteoarthritis, where the driver is mechanical wear with secondary inflammation rather than autoimmune synovitis, results are modest and inconsistent.
    Dose is the second thing people get wrong. Joint trials use 2.7 g or more of combined EPA and DHA daily, which is several times what a standard one-gram fish oil capsule provides — most people taking fish oil for their joints are taking a fraction of the studied dose. Onset is slow, typically twelve weeks or more, because the mechanism requires remodelling of cell membrane fatty acid composition.

    A systematic review and meta-analysis of randomised trials in Nutrients found marine oil supplements reduced arthritis pain, with effects clearest in inflammatory arthritis. Adequate dosing of combined EPA and DHA and at least three months of use are required.

    How It Works

    EPA and DHA incorporate into cell membrane phospholipids, displacing arachidonic acid. Because arachidonic acid is the substrate for the most inflammatory eicosanoids — series-2 prostaglandins and series-4 leukotrienes — this substitution shifts eicosanoid output towards the less inflammatory series-3 and series-5 products.
    EPA and DHA are also precursors to resolvins, protectins and maresins — specialised pro-resolving mediators that actively terminate inflammation rather than merely suppressing it. This is a genuinely different pharmacology from NSAIDs, which block prostaglandin synthesis without driving resolution. Membrane remodelling takes time, which is the mechanistic reason for the twelve-week onset, and it is also why the effect persists for weeks after stopping.

    Dosing & Protocol

    Read the back of the bottle rather than the front: what matters is combined EPA plus DHA, not total fish oil. A typical 1000 mg capsule supplies around 300 mg of the two combined, so reaching 2.7 g takes eight or nine capsules — or two to three of a concentrated product.

    Evidence

    The linked systematic review and meta-analysis in Nutrients pooled randomised trials of marine oil supplements for arthritis pain and found a significant reduction in pain relative to comparators, alongside reductions in NSAID consumption. Effects were most consistent in rheumatoid arthritis, with osteoarthritis trials showing smaller and more variable benefit. The review highlighted dose and duration as key moderators, which matches the mechanism, and noted heterogeneity in outcome measures and supplement composition across trials. The reduction in NSAID use is arguably the most useful endpoint reported, since it reflects a patient-relevant change rather than a point on a pain scale.

    Safety

    Fish oil is well tolerated at joint-relevant doses. Fishy burps, reflux and loose stools are the usual complaints; taking capsules with a meal, splitting the dose or freezing them reduces all three. Enteric-coated products help with reflux.

    Bleeding, atrial fibrillation and product quality

    High-dose omega-3 has a mild antiplatelet effect — discuss with your prescriber before combining with warfarin, DOACs or dual antiplatelet therapy, and stop around two weeks before surgery. Trials of high-dose omega-3 have reported a small increase in atrial fibrillation incidence, which is worth knowing if you have a history of arrhythmia. Choose products tested for oxidation and heavy metals; rancid fish oil is common and is pro-inflammatory, defeating the purpose.

    The interactions are almost all bleeding-related, and most are manageable with monitoring rather than avoidance. The NSAID interaction is the useful one — reduced requirement is a benefit, not a conflict.
