    Abstract

    Network meta-analysis of glucosamine and chondroitin for osteoarthritis

    Methodology

    Three-year randomized, double-blind, placebo-controlled trial with 318 patients with knee osteoarthritis. Participants received 1500mg glucosamine sulfate or placebo daily. Primary outcome: joint space narrowing measured by X-ray. Secondary outcomes: WOMAC pain and function scores, joint swelling, medication use, and radiographic progression.

    Glucosamine prevented joint space narrowing in 30% fewer patients vs placebo Average joint space loss: 0.06mm (glucosamine) vs 0.31mm (placebo) (p=0.04) WOMAC pain scores improved 28% with glucosamine vs 11% placebo Function scores significantly better in treatment group Reduced NSAID use by 45% Effects more pronounced in patients with mild-moderate OA Benefits accumulated progressively over 3 years Excellent safety profile No difference in serious adverse events

    Conclusions

    Long-term glucosamine sulfate supplementation slows radiographic progression of knee osteoarthritis and provides symptom relief. The disease-modifying effects suggest glucosamine acts beyond symptomatic treatment. Glucosamine sulfate should be considered for patients with knee OA, particularly in early-moderate stages. Effects may take 2-3 months to become apparent.
