- Take on an empty stomach, at least 30 minutes before or 2 hours after food, since digestion may denature the collagen and defeat the mechanism. Evidence is mixed. Randomised trials at 40 mg/day report reduced WOMAC pain and stiffness in knee osteoarthritis, with Lugo and colleagues finding UC-II superior to both placebo and to glucosamine plus chondroitin over 180 days, and a further trial showing benefit in healthy subjects with exercise-induced joint discomfort. The limitation is that nearly all trials come from or are funded by the ingredient manufacturer, with few independent replications and modest sample sizes.

Caveats
The evidence base is almost entirely manufacturer-funded, which is a genuine limitation for interpreting effect sizes, though the trials themselves are randomised and placebo-controlled. Exercise therapy and weight loss remain the best-evidenced interventions for knee osteoarthritis, outperforming any supplement and most drugs, and topical NSAIDs are effective with low systemic risk. Safety of UC-II: well tolerated at 40 mg/day, with occasional mild gastrointestinal upset; no upper limit is established, but there is no reason to exceed the studied dose. It is derived from chicken sternal cartilage, so it is unsuitable for vegetarians and vegans and must be avoided by anyone with chicken or egg protein allergy. The mechanism involves immune modulation through oral tolerance, so caution is sensible in autoimmune conditions and with immunosuppressants. Safety in pregnancy and breastfeeding is not established. Do not take it with food or with hot drinks. Joint pain with heat, redness, fever, rapid swelling, or symmetrical small-joint involvement with prolonged morning stiffness needs medical assessment rather than a supplement.
