The proposed mechanism is oral tolerance: small amounts of undenatured type II collagen reaching gut-associated lymphoid tissue are thought to induce regulatory T cells that dampen autoimmune attack on joint cartilage - an interesting and biologically coherent idea supported by animal work. Human trials at 40 mg/day report improvements in pain and function scores in knee osteoarthritis. No trial has measured cartilage thickness, joint space width or any structural endpoint by imaging, so cartilage regeneration or preservation remains unevidenced.

    Human trials measure symptom scores and knee extension, not cartilage structure. Supporting data are mechanistic and largely from the ingredient manufacturer.

    No supplement has been shown to regenerate or preserve cartilage in humans - glucosamine and chondroitin were tested extensively for exactly this in the GAIT trial and its imaging substudy and did not demonstrate meaningful structural benefit, and UC-II has not been tested for it at all. Claims of rebuilding cartilage should be treated as unsupported. What genuinely helps osteoarthritis is exercise therapy, particularly strengthening around the joint, and weight loss where relevant, both with strong evidence for pain and function. Safety of UC-II: well tolerated at 40 mg/day, with mild gastrointestinal upset occasionally reported and no established upper limit. It is derived from chicken sternal cartilage, so it is unsuitable for vegetarians and for anyone with chicken or egg protein allergy. Because the mechanism involves immune modulation through oral tolerance, caution is reasonable in autoimmune conditions and with immunosuppressants. Safety in pregnancy and breastfeeding is not established. Take it on an empty stomach - food may denature the collagen and defeat the mechanism. Joint pain with heat, redness, fever or rapid swelling needs urgent assessment.
