- Crystalline glucosamine sulfate is the form used in the trials that reported structural effects; glucosamine hydrochloride performed poorly in the trials that used it, and the two are not interchangeable. Chondroitin sulfate 1200 mg/day is often combined

- Take with food to reduce stomach upset. Evidence is mixed and the structural claim is the weakest part of the glucosamine story. Two long-term trials of crystalline glucosamine sulfate reported less joint space narrowing on X-ray over three years than placebo, and pooled analyses suggest a small structural effect. Against that, joint space width is an imperfect proxy for cartilage, other trials including GAIT found no structural benefit, and there is no evidence that oral glucosamine reaches cartilage in concentrations that affect proteoglycan synthesis. Cartilage regeneration is not something any oral supplement has been shown to achieve.

Safety
Caveats
Do not expect cartilage regrowth - no oral supplement has demonstrated that. What preserves joint function in osteoarthritis is weight loss (each kilogram lost reduces knee load several-fold), targeted strengthening exercise and activity modification, all of which have better evidence than any supplement. Safety: glucosamine is derived from shellfish shells, so avoid with shellfish allergy unless the product is explicitly shellfish-free. It may raise INR in people on warfarin - avoid or monitor closely. Effects on blood glucose are probably minimal but monitor if diabetic. Glucosamine sulfate preparations often carry a significant sodium or potassium load, which matters in hypertension, heart failure or renal impairment - potassium-salt forms should be avoided in chronic kidney disease. Avoid in pregnancy and breastfeeding, where safety is not established, and in children. Common effects are nausea, heartburn and diarrhoea.
