    Overview

    UC-II is the most mechanistically unusual joint supplement in common use. Everything else in the category — collagen peptides, glucosamine, chondroitin, MSM — supplies substrate or dampens inflammation. UC-II does neither. It is native, undenatured type II collagen extracted from chicken sternum cartilage under low-temperature conditions that preserve its triple-helical structure, and that structure is the whole point.

    The mechanism is oral tolerance. Intact type II collagen epitopes reaching the Peyer's patches of the small intestine are presented to gut-associated lymphoid tissue, which generates regulatory T cells specific to that antigen. Those Tregs then circulate to joints where type II collagen is exposed by cartilage wear and suppress the local autoimmune attack, reducing the inflammatory cascade that drives further degradation. This is why the dose is so strikingly small: 40 mg is an immunological signal, not a nutritional supply. Denaturing the protein by heating destroys the epitopes and the effect, which is why hydrolysed collagen peptides — a genuinely useful product — cannot substitute here.

    The clinical data are decent for a supplement. A 2009 randomised trial in knee osteoarthritis found 40 mg/day of UC-II reduced WOMAC scores significantly more than 1500 mg glucosamine hydrochloride plus 1200 mg chondroitin sulfate over 90 days. A 2016 trial in 191 subjects confirmed improvement over placebo in pain, stiffness and physical function. Smaller studies in healthy exercising adults show improved knee joint range of motion and longer pain-free exercise duration. Trials are modest in size and several were industry-funded, so treat the effect as real but not definitively established. Tolerability is excellent, with the main caution being chicken allergy and the general immunological caveat for people on immunosuppressants.
