    Creatine is a compound synthesised from arginine, glycine and methionine, stored mainly in skeletal muscle as phosphocreatine. Its function is rapid ATP regeneration: during high-intensity effort, phosphocreatine donates a phosphate group to ADP, restoring ATP within seconds. Supplementation raises muscle phosphocreatine stores by 20-40%, extending the duration of maximal effort.

    The evidence base is unusually deep. Hundreds of randomised trials and multiple meta-analyses show consistent gains in strength, power and lean mass when combined with resistance training — typically 5-15% above training alone. Effects are largest in activities lasting under 30 seconds and repeated with short rest.

    Creatine monohydrate is the form to buy. Every alternative — hydrochloride, ethyl ester, buffered forms, liquid creatine — has been marketed on claims of superior absorption, and none has outperformed monohydrate in head-to-head trials. Monohydrate is also the cheapest by a wide margin.

    Loading is optional. Twenty grams daily split into four doses for five to seven days saturates muscle faster, but 3-5 g daily reaches the same saturation in three to four weeks with less gastrointestinal upset.

    The safety picture is reassuring. Trials lasting up to five years find no adverse effect on kidney function in healthy people. The persistent kidney myth arises from creatine raising serum creatinine — a breakdown product used to estimate kidney function — without any actual change in filtration. Tell your doctor you take creatine before a kidney blood test.

    Cognitive effects are an active and promising area. Creatine appears to help most under metabolic stress: sleep deprivation, hypoxia, and in vegetarians, whose baseline stores are lower because dietary creatine comes from meat and fish.
