    Know the honest picture: the old worry that creatine causes dehydration, cramping and heat illness has been repeatedly disproven, and some trials show creatine-induced hyperhydration slightly improves thermoregulation — but other trials show no benefit.     Caveats

    Mixed verdict — the protective claim is not consistent across trials. The 1-2 kg water weight gain is a penalty in weight-bearing endurance events. Do not rely on creatine to prevent heat illness: exertional heat stroke is a medical emergency requiring rapid cooling. Creatine raises serum creatinine on lab tests — inform your clinician. Caution in chronic kidney disease. Not adequately studied in pregnancy.

    Less likely to help if

    Already heat-acclimatised athletes, endurance runners for whom the added mass offsets any benefit, and anyone with an inadequate hydration plan.
