    Overview

    Creatine's cognitive story is genuinely interesting but far less settled than its muscle story, and the difference between the two is often glossed over in marketing.
    The brain is metabolically expensive and uses the same creatine kinase system as muscle to buffer ATP. That gives a plausible route by which raising brain creatine could support tasks demanding rapid, sustained neural processing. The consistent finding across the literature is that benefits appear largest when the brain is energetically stressed - sleep deprivation, hypoxia, or in people with low dietary creatine intake such as vegetarians. In well-rested omnivores the effect on processing speed is small and inconsistently detected.

    How It Works

    Neurons use phosphocreatine to buffer ATP during bursts of firing, particularly at synapses where the cost of restoring ion gradients and recycling vesicles arrives faster than mitochondrial output can rise.
    Two features limit how much supplementation can do. The brain synthesises a substantial part of its own creatine locally, and transport across the blood-brain barrier is slow and saturable, so brain creatine rises far less than muscle creatine does on the same dose - typically a few percent rather than twenty. This is the best explanation for why cognitive effects are modest and state-dependent. Where baseline brain creatine is already adequate there is little headroom; where it is low, or where energy demand outstrips supply, a small increase can matter more.

    Diet predicts response

    Vegetarians and vegans start with lower tissue creatine and show the clearest cognitive changes. Regular meat eaters should expect little.

    In the broader literature, randomised trials in vegetarians have reported improvements in working memory and reasoning speed, and trials under sleep deprivation or hypoxic stress have found preserved reaction time and processing speed relative to placebo. Meta-analyses generally describe small effects on memory, with some signal in older adults. The qualifications are real. Trials in rested omnivorous young adults frequently find nothing, cognitive test batteries differ so much that pooling is difficult, sample sizes are small, and processing speed specifically is a less consistently studied endpoint than memory. This is a plausible modest effect in defined circumstances, not a reliable nootropic.

    Stress-dependent effect

    The clearest cognitive results come from sleep-deprived, hypoxic or low-creatine-diet participants rather than well-rested meat eaters.

    Safety

    Creatine monohydrate has an excellent safety record over trials lasting up to five years, with no adverse effect on kidney or liver markers in healthy adults.

    Slowed thinking can be a medical sign

    New or worsening difficulty with concentration and processing speed can reflect thyroid disease, B12 deficiency, sleep apnoea, depression or medication effects. Get it assessed rather than supplemented.

    Higher cognitive doses of 10-20 g are more likely to cause gastrointestinal upset and are best split. Serum creatinine rises on supplementation and can be mistaken for kidney impairment, so disclose use before renal testing. Existing kidney disease warrants medical clearance.
