    How It Works

    Concussion produces an acute neurometabolic cascade with ionic flux, hyperglycolysis and then depressed cerebral glucose metabolism, while cerebral blood flow falls. Creatine supplementation raises brain phosphocreatine stores, buffering ATP regeneration during exactly this period of energy supply-demand mismatch.

    Evidence

    What the studies say

    The most cited human evidence is a small open-label study in children and adolescents with traumatic brain injury, where six months of creatine was associated with improvements in headache, dizziness and fatigue. It was not blinded, not placebo-controlled, and has not been replicated. Magnetic resonance spectroscopy work confirms that oral creatine raises brain creatine concentrations, so the pharmacological premise holds, and separate randomised work shows creatine improves cognition under conditions of metabolic stress such as sleep deprivation. What is missing is a properly controlled trial in concussion. Given creatine monohydrate has one of the strongest safety records of any supplement and is inexpensive, the risk-benefit of a trial period is favourable, but expectations should be set accordingly.

    Caveats

    Insufficient evidence. High per-kilogram doses have not been safety-characterised for prolonged use. Raises serum creatinine on labs; avoid in chronic kidney disease. Not advised in pregnancy without clinician oversight.
