    The pump is muscle cell volumisation, and creatine causes it by a well-documented route. In a randomised controlled trial, creatine increased total and intracellular body water alongside body mass, which is precisely the physiology that makes muscles look and feel fuller.
    Worth separating two things that get conflated. Creatine produces a persistent baseline fullness from intracellular water gain, typically one to two kilograms in the first week of loading. It does not produce the acute, transient blood-flow pump that citrulline or nitrate supplements target. That baseline change is what people notice in the mirror. It arrives fast, it stays as long as you keep taking creatine, and it is intracellular rather than the subcutaneous bloat people sometimes fear.

    How It Works

    Creatine is taken into muscle by a sodium-dependent transporter and stored as phosphocreatine. Because it is osmotically active, each unit stored draws water into the cell with it, raising intracellular volume.
    That swelling is not only cosmetic. Cell volumisation is itself an anabolic signal, associated with reduced protein breakdown and increased synthesis, which is one proposed contributor to creatine's longer-term effect on lean mass. The distinction from a training pump is the mechanism. Training pump is blood and plasma moving into the working muscle for minutes; creatine fullness is water held inside muscle fibres for as long as stores stay saturated.

    Safety

    Creatine monohydrate is the most studied sports supplement in existence, with decades of trial data and no signal of kidney or liver harm in healthy people. The two real complaints are gastrointestinal upset during loading and the water-weight gain itself.
