    What the studies say

    Meta-analytic data support meaningful cortisol reduction and small testosterone increases in men; effects on DHEA, estrogen and thyroid hormones are inconsistent and sometimes undesirable. Trials in women are sparse and short.

    Ashwagandha increased salivary DHEA-S by 18% and testosterone by 14.7% versus placebo, but produced no significant improvement in self-reported fatigue, vigour or sexual well-being.

    Safety

    Caveats

    "Hormonal balance" is not a measurable endpoint - the meta-analytic support is for cortisol reduction and small testosterone increases in men. Effects on DHEA, oestrogen and thyroid hormones are inconsistent and sometimes undesirable: ashwagandha raises T3 and T4 and can push a treated hypothyroid patient towards thyrotoxicosis, and it is unsafe in hyperthyroidism. Liver injury case reports; stop with jaundice or dark urine. Avoid in autoimmune and hormone-sensitive conditions. Contraindicated in pregnancy.

    Less likely to help if

    Women seeking oestrogen or progesterone changes, men with clinically low testosterone needing medical treatment, and anyone with an untreated thyroid disorder.
