    Black Cohosh for Hormonal Balance

Black cohosh affects menopausal symptoms without measurably changing hormone levels.

    Overview

Black cohosh affects menopausal symptoms without measurably changing hormone levels.

    Verdict

    Mixed evidence

    How It Works

Rather than acting as a phytoestrogen, it appears to work through serotonergic and dopaminergic pathways involved in thermoregulation.

    Dosing & Protocol

    Typical dose

    Recommended dose
40-80 mg/day standardised extract (2.5% triterpene glycosides), with food
    Expected timeframe
    4-12 weeks

    Protocol

    form
Extract standardised to 2.5% triterpene glycosides, from a reputable brand - species adulteration is common
    step 1
Understand what this is not - trials consistently show no change in FSH, LH or estradiol, so black cohosh does not rebalance hormones
    timing
With food; split dosing helps if you get mild stomach upset
    duration
Up to 6 months, reassessed at 8-12 weeks
    titration
Increase to 40-80 mg/day in one or two doses after two weeks if symptoms persist
    measurement
A daily vasomotor symptom count, since that is the endpoint that actually responds
    starting dose
20-40 mg standardised extract once daily with food

    Evidence

    What the studies say

Trials consistently show no change in FSH, LH or estradiol, arguing against a hormonal mechanism despite symptom effects in some studies.

No studies are yet linked to both Black Cohosh and Hormonal Balance.

    Safety

    Caveats

Do not expect hormone levels to change - they do not, and measuring them will not show whether it is working. Rare but serious liver injury has been reported, including cases requiring transplant: stop immediately with jaundice, dark urine, right upper abdominal pain or unusual fatigue, and avoid with existing liver disease. Contraindicated in pregnancy and breastfeeding. Discuss with your oncologist before use in breast or other hormone-sensitive cancer. Do not use for longer than 6-12 months without review. Adulteration with other Actaea species is common - buy tested products.

    Less likely to help if

Women whose symptoms are not vasomotor, and those seeking measurable hormonal change.
