    Overview

    Selenium is a trace element the body incorporates into roughly 25 selenoproteins, including the glutathione peroxidases that neutralise peroxides and the deiodinases that convert thyroxine (T4) into active triiodothyronine (T3). Intake is almost entirely determined by soil content: a couple of Brazil nuts can exceed a day's requirement, while populations in low-selenium regions of China, parts of Europe and New Zealand fall short.

    The strongest supplementation evidence sits in autoimmune thyroid disease. Randomised trials in Hashimoto's thyroiditis have generally reported falls in thyroid peroxidase antibody titres on 200 mcg per day, though whether that translates into better thyroid function or fewer symptoms remains unsettled. In mild Graves' orbitopathy, a European trial found selenium improved eye involvement and quality of life.

    Outside deficiency and thyroid autoimmunity the case weakens considerably. The SELECT trial found no reduction in prostate cancer from 200 mcg of selenomethionine and signalled a possible increase, and the NPC trial linked long-term supplementation to a higher incidence of type 2 diabetes. Selenium is one of the clearest examples of a nutrient where more is not better: the gap between the recommended 55 mcg and the 400 mcg upper limit is small.

    Safety Information

    Potential Side Effects

    Well tolerated at the RDA. Above roughly 400 mcg daily, chronic use can cause selenosis: garlicky breath, metallic taste, brittle or shedding nails, hair loss, skin rash, GI upset and irritability. Acute overdose is a medical emergency. Long-term high-dose intake carries a signal for increased type 2 diabetes risk. Doses at the RDA in pregnancy and breastfeeding are considered appropriate; high-dose use is not.

    Contraindications

    Do not exceed 400 mcg daily from all sources without clinical supervision. Selenosis presents with garlic breath, hair and nail loss, brittle nails, a metallic taste, fatigue and irritability. High-dose long-term use has been associated with increased type 2 diabetes incidence in the NPC trial and with a non-significant increase in high-grade prostate cancer in SELECT. People with existing kidney disease should not supplement without medical advice.

    Drug Interactions

    • Cisplatin — chemotherapy can lower selenium status; supplement only under oncology guidance, as antioxidant timing may matter
    • Statins and niacin — selenium as part of an antioxidant combination has been reported to blunt the HDL rise from niacin therapy
    • Levothyroxine — selenium alters T4 to T3 conversion, so thyroid function may need rechecking after starting or stopping
    • Anticoagulants — high doses may have a modest antiplatelet effect; monitor if you are already on warfarin or a DOAC
