    Selenium for Thyroid Function Support

    Selenium is genuinely central to thyroid physiology, with the clearest supplementation benefit in autoimmune thyroiditis and mild Graves orbitopathy.

    Overview

    A meta-analysis published in Thyroid found that selenium supplementation significantly reduced thyroid peroxidase antibody levels in chronic autoimmune thyroiditis at three, six and twelve months. What the same analysis could not establish was any clear effect on clinical thyroid function or on symptoms. That gap is the whole story of this pairing. The antibody marker moves reliably; whether the person feels better or needs less levothyroxine does not follow from it. Anyone considering selenium for thyroid support should know they are treating a laboratory number with uncertain clinical meaning.

    Likely effective

    Meta-analysed evidence for reduced thyroid peroxidase antibodies at 3, 6 and 12 months in autoimmune thyroiditis. Effects on thyroid hormone levels, symptoms and disease progression remain unproven.

    The relevant population is autoimmune thyroiditis, not general thyroid support. There is no evidence that adding selenium helps a euthyroid person with no antibodies and adequate selenium status, and in that group the risk-benefit tilts unfavourably.

    How It Works

    The thyroid holds the highest selenium concentration of any organ by weight, because two selenoprotein families are essential to its work. The iodothyronine deiodinases convert T4 to active T3 and are selenoenzymes; without selenium, peripheral activation of thyroid hormone falters. The second family is the glutathione peroxidases. Thyroid hormone synthesis generates hydrogen peroxide as a required intermediate, and the gland depends on selenoenzymes to clear it. In autoimmune thyroiditis, where peroxide handling is already strained, boosting that antioxidant capacity is the proposed reason antibody titres fall.

    An antibody titre is a marker of immune activity, not a measure of gland destruction. Titres can fall while the underlying fibrotic process continues, which is a plausible explanation for the disconnect between the antibody result and the absence of functional benefit.

    Weigh the diabetes signal

    Long-term supplementation in selenium-replete populations has been associated with increased type 2 diabetes risk. Against an antibody change with no proven symptom benefit, that trade-off deserves an explicit decision.

    Selenium does not replace levothyroxine and does not allow a dose reduction. Anyone on replacement should keep their prescribed dose and their usual monitoring schedule unchanged.

    Evidence

    The Thyroid meta-analysis pooled randomised trials of selenium in chronic autoimmune thyroiditis and found significant reductions in thyroid peroxidase antibody levels at three, six and twelve months. Effects on clinical thyroid function and on patient-reported symptoms were unclear across the same trials. Heterogeneity was substantial: trials differed in selenium form, dose, baseline selenium status and whether participants were on levothyroxine. No trial has demonstrated a change in progression to overt hypothyroidism, which is the outcome that would justify routine use.

    High-dose iodine is the most common self-prescribed conflict here. Kelp and iodine supplements are widely marketed for thyroid support and can worsen autoimmune thyroiditis, which is precisely the population selenium is aimed at.
