    Evidence is insufficient. Mental clarity is not a defined or measurable outcome and has not been an endpoint in any phosphatidylserine trial. The research base concerns memory and learning measures in older adults with age-associated memory impairment, where results were positive with bovine-derived material and inconsistent with plant-derived material, plus cortisol attenuation at higher doses. Trials in healthy young adults examining attention, processing speed and executive function have generally found no benefit. The US FDA permits only a qualified claim describing the evidence as very limited and preliminary. There is no dose or protocol supported for producing a subjective sense of mental clarity.

    Positive results cluster in older adults with memory complaints, not healthy performers. Attention and processing-speed endpoints have generally been null.

    Caveats

    Subjective mental clarity or brain fog responds to identifiable causes far more than to supplements: poor sleep and untreated sleep apnoea, dehydration, alcohol, depression and anxiety, anaemia, hypothyroidism, B12 or vitamin D deficiency, uncontrolled blood glucose, chronic pain, perimenopause, long COVID, and sedating medications including antihistamines, anticholinergics, opioids and benzodiazepines. Persistent brain fog is worth investigating for these rather than supplementing around. Sleep, exercise and reducing alcohol produce noticeable change; a phospholipid capsule does not. Safety of phosphatidylserine if used for other reasons: generally well tolerated, with insomnia and gastrointestinal upset the main effects, more likely above 300 mg/day or with evening dosing; doses above 600 mg/day are not well studied. Theoretical additive bleeding risk with anticoagulants, antiplatelet drugs and fish oil, and theoretical interactions with cholinergic and anticholinergic medications such as donepezil. Avoid soy-derived products in soy allergy - sunflower-derived alternatives exist. Avoid in pregnancy and breastfeeding, as no safety data exist. New or progressive cognitive change affecting daily function warrants medical assessment.
