    Evidence is insufficient. Mental clarity is not a validated outcome, and magnesium has not been shown to improve cognition in people with adequate status. Magnesium L-threonate is heavily marketed on this basis, but the supporting work is largely rodent studies showing increased brain magnesium and improved synaptic density and memory; human trials are few, small, industry-linked and short, and none establishes a clinically meaningful cognitive benefit. Observational data associate low magnesium status with poorer cognitive performance and higher dementia risk, but that is confounded by overall diet and health. A real case exists for correcting deficiency, since magnesium depletion causes fatigue, poor concentration and irritability - but that is treating a deficiency, not enhancing cognition.

    What the studies say

    Human data are limited to small threonate trials in older adults with subjective cognitive complaints and to observational deficiency associations. No adequately powered trial supports a nootropic effect in healthy adults.

    Caveats

    Brain fog deserves investigation rather than supplementation: sleep deprivation, sleep apnoea, iron or B12 deficiency, hypothyroidism, depression, anxiety, perimenopause, alcohol, medication side effects and post-viral syndromes all cause it, and nearly all are testable. Persistent or progressive cognitive decline needs medical assessment. Note also that magnesium L-threonate typically costs several times more than well-absorbed conventional forms while delivering less elemental magnesium per capsule. Safety ceiling: the tolerable upper intake level for supplemental magnesium is 350 mg/day for adults, above which diarrhoea, nausea and cramping are expected; the limit excludes dietary magnesium. Magnesium may add to the effects of sedatives and alcohol, and at higher doses can cause drowsiness rather than clarity. It reduces absorption of tetracycline and quinolone antibiotics, bisphosphonates and levothyroxine - separate by 2-4 hours. People with impaired kidney function must not supplement without medical advice. Discuss use in pregnancy with your clinician.
