How to Measure
Start by quantifying sleep — duration, timing and consistency over two weeks — and screen for obstructive sleep apnoea if there is snoring, witnessed pauses or unrefreshing sleep. Then run bloods that cover the treatable causes: full blood count and ferritin, B12 and folate, TSH, vitamin D, HbA1c, and renal and liver function. Review every medication and supplement for anticholinergic or sedating properties. Screen for depression and anxiety, which cause genuine measurable cognitive impairment rather than merely feeling like it. Formal cognitive testing belongs with a clinician when impairment is progressive, affects daily function, or is noticed by others rather than only by you.

Review medications with your prescriber for anticholinergic and sedating agents. Treat depression, anxiety and thyroid disease where present. Then address the durable levers: regular aerobic exercise, which has the best evidence of any intervention for cognitive ageing; blood pressure, lipid and glucose control, since vascular health drives long-term cognition; and reduced alcohol.
