    Overview

    Ashwagandha is the best-supported supplement for stress and anxiety, which is a statement about a weak field as much as about ashwagandha. Multiple randomised placebo-controlled trials at 300-600 mg of standardised root extract show reductions in perceived stress scores and in serum cortisol, consistently but modestly. The effect is real and it is small. It suits people with everyday stress and mild anxiety; it does not treat a diagnosed anxiety disorder, and it should not delay proper care for one.

    Modest, not transformative

    Expect a reduction in stress reactivity over six to eight weeks, not the disappearance of anxiety. Trials measure differences in questionnaire scores, not remission.

    Six to eight weeks is the trial period. People with low baseline stress see little, and anxiety driven by hyperthyroidism, stimulants, or alcohol will not respond to an adaptogen — those causes need addressing directly.

    How It Works

    Ashwagandha's withanolides act on the hypothalamic-pituitary-adrenal axis, reducing its reactivity to stressors. The most consistently replicated biochemical finding across trials is a reduction in serum cortisol, typically in the region of 15 to 30 percent, alongside reduced subjective stress.
    There is also a GABAergic component. Withanolides show GABA-mimetic activity at GABA-A receptors in preclinical work, which offers a plausible explanation for the anxiolytic and mildly sedating effects reported and for why some people notice improved sleep before they notice anything about stress. Antioxidant effects in neural tissue may contribute over longer periods.

    Dosing & Protocol

    Three hundred to six hundred milligrams daily of a root extract standardised to withanolide content — usually 5% or expressed in milligrams of withanolides. The well-known branded extracts used in trials are standardised this way; unstandardised root powder at the same weight is not equivalent. Split dosing morning and evening suits most people, or take it all in the evening if you find it sedating or your main complaint is sleep. Take with food to reduce the gastrointestinal upset that is the commonest reason people stop.

    Meta-analyses of randomised controlled trials report significant reductions in perceived stress and anxiety scores versus placebo, with concurrent reductions in morning serum cortisol. The consistency of the cortisol finding across independent trials is the strongest part of the evidence.
    Effect sizes vary widely between trials, many studies are small, short and conducted by manufacturer-affiliated groups in a single region, and heterogeneity in extracts and outcome scales complicates pooling. There is also a plausible risk of publication bias in a commercially active area.

    Safety

    Gastrointestinal upset, drowsiness and loose stools are the common effects, mostly mitigated by taking it with food. The signal that deserves attention is hepatotoxicity: rare but well-documented cases of drug-induced liver injury have been reported, some serious. Ashwagandha also raises thyroid hormone levels, which is relevant in thyroid disease, and should be avoided in pregnancy.
